You need to provide SSA witht he appropriate documentation for your child. All of these documents need to prove the following: The child’s identity. This is a document that shows your child’s name, information of identification such as birth date, age or parents’ names. Also, it is preferable if you show any of these documents with a photograph. What works for this is a State-issued nondriver’s identification card. A United States Passport. Adoption Decree. Doctor, clinic or hospital record of the child. Religious record. School or caycare center record. And finally, any school identification card.
You also need to show your child’s age through the following documents: A religious record made before the age of 5 that shows the child’s date of birth. A United States Hospital record of birth or a U.S. Passport. All of these documents must be current and be either an original or a certified copy from the issuing agency.
